Eligibility Requirements for ID Card shipped directly to cardholder - Shipping only available when replacing ID Card.
1. Existing USID Cardholder
2. Photograph has been taken in the last 12 years
3. Benefits extend at least 30 days in the future
4. Cardholder's associated Residential or Mailing Address is a US address and not a P.O Box (see My Profile)
5. Sponsor must have email address with permission to allow DoD correspondence (see My Profile)
6. Family Member must have email address with permission to allow DoD correspondence (see My Profile)
7. Sponsor or family member record is not terminated
8. Sponsor or family member record is not locked
9. Access to Cardholder record is not restricted
